Social Care Navigator (Case Manager) at Virginia Garcia Memorial Health Center and Foundation

Florencia Valle-Miller has a diverse range of work experiences spanning various roles and industries.

Starting in 2000, they worked at San Francisco School Volunteers (SFSV) as a School/Community Coordinator, where they recruited, trained, and placed volunteers in public school classrooms. Additionally, they provided Spanish translation of educational materials for parents and facilitated literacy trainings for parents in both Spanish and English.

In 2005, Florencia joined the Legal Aid Society-Employment Law Center (LAS-ELC) as an Administrative Coordinator in the Direct Services Program. Florencia'sresponsibilities included organizing the Workers' Rights Clinic, providing general administrative support, and managing client intake and volunteer attorney databases.

From 2014 to 2014, they worked as a Translator/Interpreter at the San Francisco Unified School District, translating various documents from English to Spanish and vice versa. Florencia also provided interpretation services at school meetings, conferences, and events.

Starting in 2015, Florencia worked at Outside In as a Clinic Support Specialist/Lead Spanish Interpreter. Florencia'srole involved interpreting for medical appointments, coordinating interpreter schedules, and relaying patient lab results. Florencia also assisted with patient care coordination.

In 2017, they joined the Immigrant and Refugee Community Organization as an English-Spanish translator and interpreter. During their time there, they provided translation services and interpretation for various purposes.

Most recently, since 2018, Florencia has been employed at the Virginia Garcia Memorial Health Center and Foundation. Florencia initially worked as a Panel Care Coordinator, coordinating care for patients with complex health problems or psychosocial issues. Currently, they serve as a Social Care Navigator (Case Manager), assessing and addressing patient needs related to their resilience, access to outpatient services, and social determinants of health. Florencia provides skills training, case management services, and links patients to community resources to address their medical, mental health, and psychosocial needs. Florencia also collaborates with primary care teams to outreach and assist patients with complex health or psychosocial needs.

Florencia Valle-Miller has a diverse educational background. Florencia earned a Master of Social Work (MSW) degree from the Boston University School of Social Work, completing their studies from 2022 to 2025. Prior to that, they obtained a Certificate in Spanish Legal/Court Interpretation from the San Francisco State University, College of Extended Learning. During the period of 2000 to 2002, they specialized in Interpretation (English-Spanish) through this program. Moving further back, they received their Bachelor of Arts (B.A.) degree in Communications from Wesleyan College between 1995 and 1997. Lastly, Florencia pursued Journalism at Universidad Finis Terrae from 1989 to 1992, although the specific degree attained is not available. In addition to their degrees, they hold a Spanish Certified Healthcare Interpreter (CHI) certification from the Certification Commission for Healthcare Interpreters (CCHI), which they obtained in October 2017.
