Tara Moore, MSW, CCM has a diverse work experience in the social work field. Tara currently works as a Transitional Care Navigator and Social Worker at Transitions LifeCare. Prior to this, they served as the President of the Board of Directors at the North Carolina Down Syndrome Alliance.

Tara also has several years of experience at Alliance Health, where they held various roles. Tara started as an I/DD Care Coordinator in January 2018 and transitioned to become an I/DD Care Navigator in July 2019. Tara then progressed to the role of IDD Care Manager I, which they held until April 2022.

Before their time at Alliance Health, Tara worked at Durham Exchange Club Industries, Inc. as a Lead Coordinator from January 2016 to January 2018. Tara also served as a Self Advocacy Advisor at The Arc of the Triangle, Inc. from February 2013 to January 2018 and briefly held the position of Individual Services Supervisor from April 2015 to December 2015.

Tara's work experience demonstrates their commitment to supporting individuals with disabilities and advocating for their rights. Tara has a strong background in care coordination and navigation, as well as experience in board leadership.

Tara Moore, MSW, CCM has a Master of Social Work (M.S.W.) degree in Community, Management, and Policy Practice from the University of North Carolina at Chapel Hill, which they obtained from 2011 to 2012. Prior to that, they completed their Bachelor's of Social Work degree at Appalachian State University from 2007 to 2011. Additionally, Tara holds the certification of Certified Care Manager (CMC) from the National Academy of Certified Care Managers, which they obtained in June 2021.
