Jordan Neal is an accomplished public health professional with a strong background in population health management and community health services. Currently serving as the Director of Population Health at AccessHealth Community Health Center since June 2023, Jordan previously held the position of Senior Population Health Department Manager at the same organization. Prior experience includes roles as Pregnancy Medical Home Program Coordinator at REACHUP, INC. and Healthy Families Clinical Supervisor at Pasco Kids First. Additionally, Jordan contributed to REACHUP, INC. as a Perinatal Support Outreach Worker and worked as a Healthy Transitions Care Coordinator at Carey Counseling Center, Inc. Jordan holds a Master of Public Health with a focus on Epidemiology and Global Communicable Disease from the University of South Florida, along with a Bachelor of Arts in Psychology from Berea College.
