Wendy Prystal-Monforte is a seasoned healthcare professional with extensive experience in case management and home care. Currently serving as the Lead Case Manager at ConcertoCare since May 2022, Wendy also holds the position of Patient Care Manager at WILLCARE since January 2021. Wendy's background includes roles as a Case Management Nurse in a hospital setting since July 2019 and as a Home Care Supervisor at Hudson Valley Home Care from May 2014 to January 2019. Wendy holds a Bachelor’s degree in Registered Nursing from SUNY New Paltz, completed between 1996 and 1999.
