Dana Knapp has extensive experience in social services and case management within healthcare settings, currently serving as Clinical Liaison and previously as Director of Social Services at The Center at Northridge since August 2016. Prior to this role, Dana held the position of Director of Social Services at Western Hills Health Care Center from October 2014 to August 2016, where responsibilities included case management and discharge planning for a skilled rehabilitation unit. Dana's experience also includes serving as Director of Social Services at Littleton Care and Rehabilitation and Broomfield Skilled Nursing and Rehabilitation, with employment dates ranging from April 2011 to October 2014. Educational qualifications include a Master of Science in Counseling with a Gerontology Certificate from National Louis University and a Bachelor of Social Work from Illinois State University.
