Rachel Choate is a Registered Nurse and Family Nurse Practitioner-Certified with extensive experience in healthcare settings. Rachel has held various leadership roles, including Director of Advanced Practice Nursing at St. Croix Hospice and Director of Clinical Programs and Palliative Care at CoxHealth at Home. With a background in ICU nursing at Mercy, Rachel also served as Chief Clinical Officer and Administrator at Transitions Hospice. Rachel has a Bachelor's Degree in Registered Nursing and completed the MSN FNP Program at Cox College.
