The Community and Care Services team at SKIP of NY is dedicated to providing comprehensive support and services to severely sick and developmentally disabled children. This multi-disciplinary group includes Medicaid Service Coordinators, Care Managers, Respite Specialists, and Community Habilitation Workers, among others, working collaboratively to ensure that these children can transition from hospitals and institutions to live at home with their families. Through service coordination, advocacy, and hands-on problem solving, they strive to offer the best possible care, enhancing the quality of life and fostering a nurturing environment for the children and their families.
