Alli Chrzanowski is the Director of Help-at-Home at LifeCare Alliance. Alli has been with the company since July 2017, first as a Home Care Manager and then as Director. Alli has been recognized as an outstanding patient-centered care provider, winning the Oncology Navigator of the Year award in June 2019 from the Cancer Support Community.
As Home Care Manager, Alli oversaw the Home Care Program which provided a variety of services to Cancer Clinic clients including financial assistance, help at home, medication coverage, nutritional support and transportation assistance to medical appointments. Alli also managed the Pantry Coordinator and the Groceries to Go program. Alli supervised 1-2 interns annually from accredited colleges through the Cancer Clinic.
In their role as Director, Alli continues to assess and monitor eligibility for Columbus Cancer Clinic clients by reviewing and completing an average of 65 applications per month. Alli also leads the Adopt a Family program on an annual basis, coordinating and matching 70 to 85 client families with 50-60 citizen and corporate donors to ensure the client families receive gifts and experience the holiday spirit in their time of need. In addition, Alli creates new processes to improve logistics of the clinic, such as updating applications and the steps that are taken to complete an application.
Alli provides financial assistance to clients by gathering information and analyzing their specific situations in order to effectively apply for grants on their behalf. Alli also builds and maintains relationships with partnering agencies through
