Saralin Tiedeman is a seasoned healthcare professional with a robust background in population health management and clinical performance. Currently serving as a Senior Healthcare Innovator and Population Health Management Chief Strategist at Independent Health since April 2020, Saralin has previously held key positions such as Director of Health Information Systems and Population Health at Horizon Health Services, and Director of Population Health Clinical Performance at Millennium Collaborative Care. Saralin's expertise encompasses the implementation and monitoring of population health initiatives aimed at improving patient outcomes, as well as fostering partnerships across medical, behavioral, and social health sectors. Saralin began a career in healthcare as an Occupational Therapist at Kaleida Health before obtaining a Master of Science in Occupational Science and a Bachelor of Arts in Psychology from the University at Buffalo.
