Lisa Thomas is a seasoned professional in the healthcare industry, currently serving as a Transitional Care Coordinator at Horizon Home Health and Hospice since April 2022. Prior to this role, Lisa held positions as Home Health/Hospice Area Manager and Hospice Area Manager at Encompass Health from March 2020 to April 2022.
