Rhiannon Picariello is a dedicated healthcare professional with extensive experience in patient support and population health. Currently serving as a Patient Support Associate at Galileo since January 2020, Rhiannon coordinates care by managing referrals and triaging patient concerns through various communication channels. Rhiannon's previous role as a Population Health Support member involved closing HEDIS care gaps and enhancing medication adherence, while also planning educational health fairs for rural populations. In an earlier position as a Health Advocate, Rhiannon focused on improving access to healthcare for underserved communities through assistance programs and health coaching. Prior experience includes serving as a Certified Nursing Assistant at ComForCare, providing essential support for clients in their homes or skilled facilities. Rhiannon completed education at Wake Technical Community College from 2017 to 2019.
