is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. FORM provides high-quality expert care and leverages technology to enhance the patient experience. All FORM patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. We hold ourselves to the highest standards of clinical care, and to treating every individual with empathy and respect.

About the Role:

Form Health is looking to add a talented creative leader to our team to champion and oversee the creative direction of the Form Health brand. The Creative Director will work closely with the Head of Marketing to guide and oversee the brand’s visual identity to support overall brand positioning and equity. The ideal candidate will have a strong background in design along with experience in art direction and creative team leadership. S/he will be passionate about applying creative and visual design strategies to contribute to the achievement of business goals. 

This is a full-time, remote opportunity and can be located anywhere in the U.S.

Key Responsibilities

  • Partner with the Head of Marketing to guide brand strategy. Maintain and evolve strong brand guidelines including logo, color palette, fonts, design language, and tone of voice. Ensure all work, whether developed internally or externally, complies with brand guidelines. 
  • Develop compelling creative “big ideas” for brand campaigns and marketing communications that leverage differentiated brand positioning, can be extended across marketing channels, and consider multiple brand targets (B2C, B2B, and B2B2C).
  • Bring the brand to life across all patient, commercial, and provider touchpoints (web, digital advertising, email, print, sales materials, photography, illustrations, video, etc.) with hands-on design work and managing contractors and agencies as needed.
  • Contribute to content marketing strategy by delivering high-impact creative assets and sharing innovative, insight-driven creative ideas. 
  • Engage external resources as needed to support creative vision (photography, videography, animation, etc.). Manage external partners to ensure high quality work addresses project brief and is delivered on schedule and within budget.
  • Stay up to date with design strategies, consumer trends, and emerging technologies and identify opportunities for Form Health to be innovative and evolve.

We seek applicants who have: 

  • 10+ years of design experience with a minimum of 2 years of creative leadership experience, preferably with a brand that has both consumer and commercial audiences.
  • Experience in digital healthcare as well as in leading creative strategy for a brand with multiple targets (patient, employer, payer, provider, etc).
  • Deep understanding of digital creative best practices to drive engagement & conversion.
  • Low ego and open to constructive feedback; leverages feedback to develop creative approaches that drive business results.
  • Highly organized and disciplined; manages timelines to deliver against deadlines.
  • Collaborative approach with experience partnering with Marketing, Sales, Creative, and Product colleagues.
  • Effective communication style - clearly presents ideas and listens intently to understand needs and feedback.
  • Balance of strategic thought leadership and willingness to “get your hands dirty” by digging into the details of execution from campaign ideation to simple layout design.
  • Prior startup experience and a passion for working in scrappy, high growth startups.

Base Annual Salary
$150,000$180,000 USD
