Samantha Meas is a Clinical Integration Nurse Case Manager at Fallon Health since March 2022, focusing on patient education, care coordination, and health status monitoring. Previously, as a Homecare Registered Nurse at Elder Services of the Merrimack Valley, Inc. from October 2016 to March 2022, Samantha conducted in-home assessments and collaborated with healthcare teams to create individual care plans for elderly clients. Experience includes providing direct care to children and adults with complex medical needs at New England Pediatric Care in 2016, supporting educational activities as a Teacher Assistant at Community Teamwork from 2014 to 2016, and delivering in-home patient care as a Home Health Aide at Right At Home from 2013 to 2014.
