Judy Bearer possesses extensive experience in social work and healthcare management, with a focus on supporting individuals transitioning from hospital settings to home and community-based care. Currently serving as an Integrated Care Assessor and Transition Coach at Direction Home Akron Canton Area Agency on Aging & Disabilities since December 2015, Judy Bearer aids people with serious illnesses in navigating the challenges of hospital discharge through the Community-Based Care Transitions Program. Previous roles include Ohio Home Care/MyCare Ohio Waiver Care Manager at Area Agency on Aging 10B, Inc., Social Worker positions at Concordia at Sumner, Akron General Edwin Shaw Rehab, and various care centers, as well as PASSPORT Case Manager at Area Agency on Aging 10B. Judy Bearer holds a Bachelor’s degree in Psychology from The University of Akron.
