Cydney Price, LCSW, currently serves as a Collaborative Care Clinician at Concert Health, where responsibilities include screening and assessing patients for mental health and substance abuse disorders, documenting patient progress, and providing brief interventions using evidence-based techniques. Previous roles include Social Worker at DaVita Kidney Care, where Cydney assisted patients with transplant referrals, and Outpatient Counselor at Gateway Community Services, Inc., managing a caseload and developing treatment plans.
