Erin LeCrone has extensive experience in care management and health education, currently serving as a Care Manager at Colorado Access since January 2015, where complex care coordination and risk factor identification for preventable hospitalizations are key responsibilities. Prior to this role, Erin worked as a Health Educator at WellPoint from January 2011 to July 2014, focusing on health assessments and developing personalized SMART Goals for members. Erin began a career in health coaching at Healthways from January 2008 to January 2011, managing a large caseload and employing Motivational Interviewing techniques to facilitate lifestyle changes. An early internship at Coconino County Community Services Department provided valuable experience in senior nutrition programs and client assessments. Erin holds a Bachelor's degree in Social Work from Northern Arizona University, obtained in 2007.
