Natalie Thies is a dedicated Medical Social Worker at Citrus Valley Health Partners since April 2014, focusing on population health. In previous roles, Thies worked as a Discharge Planner at Citrus Valley Medical Centers, coordinating post-discharge services and collaborating with interdisciplinary teams to enhance patient care. Thies gained experience as an MSW Intern at PIH Health, providing counseling and facilitating advance care planning, along with roles in social services and education, including coaching competitive cheerleading at Pomona Catholic High School and tutoring in Azusa Reads, Writes and Counts. Natalie holds a Master’s Degree in Clinical/Medical Social Work and a Bachelor's in Social Work, both from Azusa Pacific University.
