Susana Fernandez-Denton is a seasoned professional in human services with extensive experience in geriatric support and case management. Currently serving as a Geriatric Support Specialist at Boston Senior Home Care since July 2020, Susana previously held multiple roles at Crossroads Rhode Island, including High Acuity Case Manager and Rapid Rehousing Case Manager, where responsibilities included safety planning, case management, and advocating for residents. Prior experience includes serving as Program Coordinator/Assistant Program Director at MAP Behavioral Health Services, where Susana supervised staff and provided counseling and testing for HIV and Hepatitis C. Additionally, roles at Cross Roads of Rhode Island as a Case Advocate and at MAP Alcohol & Drug Rehab Services as a Group Case-Manager focused on support for vulnerable populations. Susana holds a Bachelor of Science degree in Human Services from Springfield College of Rhode Island.
