Jason Nietzschmann is an experienced health care professional currently serving as a Health Homes Care Manager at Access: Supports for Living since October 2022, where responsibilities include collaborating with patient families, monitoring recovery, and facilitating access to health services. Previously, Jason held roles as a Coordinator with the Department of Social Services, focusing on housing stabilization, and as a Health Homes Care Manager at HONOR, providing health guidance to Medicaid recipients with chronic conditions. Additional experience includes work as a Resident Counselor and Community Outreach Worker at HONOR, supporting homeless youth and community members, respectively. Jason holds a Master of Public Health from CUNY Graduate School of Public Health and Health Policy and a Bachelor of Arts from SUNY Geneseo.
