Connie Rhodes MSW is an experienced social work professional with a robust background in managing and coordinating social services across various healthcare settings. Currently serving as an Engagement Specialist at Vital Decisions since May 2022, Connie assists members and their families with Medical Advance Care planning and ensures that care experiences align with individual preferences. Previously, as a BPCI Program Case Manager at Center For Sports Medicine & Orthopedics, Connie managed care for joint replacement surgery patients and collaborated with multidisciplinary teams to advocate for optimal patient outcomes. Past roles include Director of Social Services at multiple skilled care facilities, where Connie provided psychosocial evaluations, emotional support, and effective discharge planning. With a Master of Social Work degree from Southern Adventist University, Connie has developed strong skills in patient advocacy, interdisciplinary collaboration, and community resource utilization throughout a career spanning over two decades.
