Makoto Hang is a skilled pharmacist with extensive experience in various healthcare settings. Currently serving as an MTM Pharmacist at UCare since September 2022 and as a Clinical Staff Pharmacist at Minnesota Community Care since July 2021, Makoto focuses on helping patients manage chronic diseases through collaborative practice agreements. Previous roles include a Staff Pharmacist at CUB PHARMACY SUPERVALU PHARMARCIES INC and a Pharmacy Intern at PHALEN FAMILY PHARMACY LTD, where responsibilities included performing comprehensive medication reviews and educating patients on their medications. Makoto has also interned at North Memorial Health, Healtheast St. Johns Hospital, and worked as a Pharmacy Technician at Health Partners and CVS/pharmacy. Makoto holds a Doctor of Pharmacy (PharmD) from the University of Minnesota, complemented by a Bachelor’s degree in Biology and an Associate of Arts degree.
