Haley Herr has over nine years of nursing experience, currently serving as a Transition of Care Registered Nurse at The South Bend Clinic since June 2015. In this role, Haley acts as a patient advocate and clinical liaison, coordinating the transition of patients to the next level of care while collaborating with various healthcare facilities to ensure continuity of care and reduce readmissions. Prior experience includes serving as a Surgical Nurse at Saint Joseph Health System and an Organ Transplant Nurse at Indiana University Health. Haley also worked as an Applied Behavior Therapist at The Applied Behavior Center for Autism. Haley earned a Bachelor of Science in Nursing from Indiana Wesleyan University in 2013.
