Jo has been with Springwell in a variety of roles since 2005. Jo manages all of the collaborative relationships with local hospitals, physicians’ groups and Accountable Care Organizations to promote improved care transitions and enhanced supports for frail and socially vulnerable patients. Jo also launched and directs Springwell’s Private Care Management Program. She has an MSW from Boston College.
