Nichole Swackhamer

Nichole Swackhamer has a diverse work experience, starting in 2009 when they became the Director of Clinical Services at OCS. In 2010, they became the owner of OCS, where they provided various services in the field of occupational therapy. Nichole also worked as a Certified Case Manager at OCS. In 2022, they became the President of OCS. Outside of OCS, Nichole was an active member of the Junior League from 2013 onwards, focusing on community improvement. OCS is a medical and vocational case management company that has been around for over 30 yrs. We are certified as a women owned small business through WBENC and are an approved rehabilitation facility through the State of Michigan. OCS home office is in Bay City, MI with case managers spread throughout the State. Our coverage area is the entire State of MI including the UP, OH, IN, IL, Eastern WI and NE. Our staff includes certified case managers (CCM), RN's, OT, and vocational counselors as well as a brain injury specialist, medical record review specialist, a certified prosthetist and a certified life care planner. All of our case manager’s have been in the healthcare field with hands on experience for a min of 5 yrs. We tailor our programs to meet your needs; offering as much, or as little involvement as you need. The case manager’s coordinate care, services, and health education for the client. We develop, implement and monitor individual rehabilitation plans and communicate these plans to patients, families, providers and staff. This ensures quality patient care and return-to-work status as quickly and as cost effectively as possible. OCS also provides health & safety evals, job site & ergonomics analysis, SSI & SSDI application assistance.
