Jay V. Patel is currently an Advisor at illuminate.health, and previously held the position of Clinical Transformation Officer at Seniorlink from April 2015 to March 2019. In this role, Jay led a multi-disciplinary clinical team to design, pilot and bring to market remote-based coaching programs; pilot results showed a nearly 20% reduction in hospitalizations, 50% reduction ER visits and 10% increase in caregiver activation.
Shaping a culture of quality improvement and person-centered care that positively impacted social determinants of health as a core part of the model of care, the organization achieved a three-year accreditation for NCQA in Case Management and was the first in the nation to be jointly accredited for Case Management with Long Term Support and Services (LTSS) Distinction.
Building and implementing an inventory of over 200 clinical protocols that addressed chronic conditions and social determinants of health, Jay resulted in improved care team efficiency.
Jay V. Patel received their MBA from Harvard Business School and their Pharm D. from the University of the Sciences in Philadelphia in the field of pharmacy.
