Corynth Torres, BSN, RN, CCM, is a seasoned nursing professional with extensive experience in care management and patient support. Currently serving as the Northwest Care Management Lead at Horizon NJ Health since February 2015, Corynth is part of a pioneering team focused on enhancing service delivery for Managed Long Term Services and Supports. Previously, Corynth worked as a Charge Nurse at Fresenius Medical Care, where responsibilities included staffing, compliance, and direct patient care.
