The Support Services Team at Evergreen Health ensures seamless delivery of ancillary care, including case management, housing assistance, and patient advocacy, to enhance the overall well-being of patients, particularly those living with HIV, STDs, hepatitis C, and within the LGBTQ and drug user communities. Team members like the VP of Supportive Services, VP of Care Coordination, and Director of Support Services collaborate to integrate these essential services into the patient care continuum.
