Tricia Lazare is an experienced professional in the field of community resources and self-determination, currently serving as Self-Determination Manager at Inclusa since March 2012 and as Community Resources Manager at Community Care Connections of Wisconsin since September 2013. Additionally, Tricia holds the position of President at the Respite Care Association of Wisconsin, providing guidance and support to enhance organizational effectiveness. Tricia has a strong background in quality improvement and advocacy for long-term care, demonstrated through roles as Provider Relations Coordinator for Adult Family Homes and Social Work Service Coordinator at Community Care of Central Wisconsin. Tricia's education includes a Master of Science in Human and Community Resources and a Bachelor of Science in Sociology, both from the University of Wisconsin-Stevens Point.
