Sarah Weis, LCSW, is a dedicated social worker with extensive experience in behavioral health and medical social work. Currently serving as a Behavioral Health Care Coordinator at CareOregon since March 2021 and as a Hospice Medical Social Worker at Signature Healthcare at Home since November 2018, Sarah has previously worked as a Behavioral Health Specialist at ROGUE COMMUNITY HEALTH and a Medical Social Worker at Amedisys. With a foundation in child and family therapy as a QMHP at Jackson County, Oregon, and experience in crisis intervention at Family Violence Law Center, Sarah's background also includes roles in mental health therapy and social work internships. Sarah holds a Master’s Degree in Clinical/Medical Social Work from San Jose State University and a Bachelor of Science in Psychology from Southern Oregon University.
