Sofia Warner serves as the Medical Director of Value Based Care at Boston Medical Center since September 2020, leading initiatives to reduce total cost of care and improve care coordination, particularly in chronic disease management. Prior roles include Internal Medicine Physician in Primary Care and Interim Associate Medical Director of Primary Care, emphasizing practice operations and clinician support, as well as Associate Medical Director of Population Health, where responsibilities included managing clinical programs for MassHealth and Medicare contracts. Sofia Warner co-founded HumanFirst in April 2017 and has previous experience as an Internal Medicine Resident at Massachusetts General Hospital and a Summer Consultant at The Boston Consulting Group. Educational qualifications include an MBA from Harvard Business School and an MD from Harvard Medical School, alongside a Bachelor of Arts from Wesleyan University.
