Holly C Young is a dedicated healthcare professional with extensive experience in case management and patient care coordination. Currently serving as a Case Manager at Adventist Health since October 2021, Holly previously held the position of Patient Care Coordinator at Kindred at Home, where responsibilities included pre-discharge planning and documentation of patient information.
