Phyllis Smith is a dedicated RN Case Manager at Ascension Health Information Systems since December 2018, where responsibilities include performing clinical assessments for various medical diagnoses, managing lab draws, and developing patient-centered care plans. Prior to this role, Phyllis served as a Case Manager at Ascension at Home for one year, focusing on clinical case management. Phyllis holds a Bachelor’s degree in Nursing and Chemistry, as well as a BSN and MSN from the University of Michigan-Flint, completing studies between 2002 and 2006.
